Refer a Client for Victims-of-Crime Psychology Support
Use this form to request counselling, a provider quote, a report-scope review or service coordination for a person with a current FAS pathway or an existing VOCAT matter. Please confirm the client’s authority before sharing personal information.

Referral types

The practice can review enquiries for:

  • psychology counselling and provider availability

  • a counselling quote or provider details

  • a treating/counselling report requested or pre-authorised by FAS

  • a non-treating assessment/report enquiry, subject to scope and independence

  • coordination with an existing VOCAT award or future-expense arrangement

Before you refer

Please have the following information where available:

  • the client’s consent/authority and safe-contact instructions

  • whether the matter is FAS, existing VOCAT or uncertain

  • Individual Application Number or VOCAT reference

  • approval/request type, sessions and relevant dates

  • report questions, author role, due date and pre-authorised fee if a report is requested

  • accessibility, interpreter or communication needs

Do not attach sensitive records

The first referral form does not accept police statements, court material, identity documents, medical records or detailed offence accounts. If Meyer Institute can review the referral, we will provide a scoped document request and approved secure transfer method.

Referral response

Submitting a referral does not confirm acceptance, availability, funding or a report due date. The practice will review clinical fit, capacity, authority, scope and payment requirements, then respond using the nominated professional contact details.

FAQs

Can I refer before FAS approval?

Yes, for a preliminary fit or information enquiry. Funded work begins only after the required approval and payment arrangements are confirmed.

What consent do you need?

Confirm that the client, parent or legal representative authorises you to share the information and ask Meyer Institute to make contact or liaise. The practice will complete its own consent process if the referral proceeds.

Can I email records after submitting?

Please wait for a scoped request and secure transfer instructions. Do not send sensitive attachments to a general mailbox.

Can you accept an urgent report?

A due date is not accepted until a clinician reviews scope, capacity, records, assessment requirements and written fee approval.

Will Meyer provide an outcome opinion in advance?

No. Any clinical or report conclusion must remain the psychologist’s independent professional opinion.

Submit a concise, consent-backed referral. Do not include a detailed offence narrative or attach sensitive records.
